Section X: How To Apply For Benefits
Application forms and income tax withholding forms are available from the Plan Administrative Office. You will need to provide evidence of your age and marital status. If you are divorced, you will need to provide a copy of the court order(s) terminating your marriage and approving your property settlement.
You should complete the application procedure no more than 90 calendar days before you want your benefits to begin.
All applications for benefits and elections should be sent to the Administrative Office. The Plan Administrator determines your eligibility and calculates your benefits, if any, as soon as possible after receiving your completed application and required supporting documentation.
