Section VII: Dental Coverage
As a retiree covered by this Fund, you have the option of selecting between
two different dental programs offered through Delta Dental: Delta AdapTable
and DeltaCare/PMI. If you are covered by Medicare and belong to an HMO, you
may select either program at no charge. Your HMO may also have a low- or no-cost
dental plan available.
If you do not have Medicare or you have Medicare but are enrolled in The Union
Labor Life Insurance Company program and you choose to enroll in the dental
plan, you must pay for it yourself through a pension check deduction. The premiums
for the two plans can change annually, so you should contact the administrative
office to determine the current cost and benefits. Note: once you enroll in
the dental plan you must continue for at least two years. If you disenroll in
the dental plan, you will not be allowed to re-enroll at a later date, except
as otherwise required by law.
Delta AdapTable
Under this program, you can use any dentist. The Plan pays benefits according
to a schedule. The patient is responsible for charges which exceed the schedule.
Claims are processed by Delta Dental.
DeltaCare/PMI
Under this program you must select a DeltaCare dentist and use him/her for all
your non-emergency dental care. If you use a non-DeltaCare dentist without prior
authorization from DeltaCare, services will not be covered except in an emergency.
Under most circumstances there are no claim forms, and services are covered
based on fixed participant copayments.
