These benefits are provided by the Plan for all eligible participants except self-pay participants. (See the Extended Coverage section for more information.) | | |

Life Insurance Benefit
| Employee - $25,000
Spouse and Dependent Child - $2,500

Terminal Illness | Active members who have been continuously covered for two years and are diagnosed with a terminal illness may be able to receive an accelerated benefit of up to 50% (but not less than $5,000) of the life insurance benefit. | | |

Accidental Death & Dismemberment | In addition to the amount payable for life insurance, an additional benefit is available if the participant suffers death or dismemberment is caused by an accident. The full AD&D benefit of $5,000 is paid for the loss of life or for any two hands, feet, eyes, or combination. Half the benefit ($2,500) is paid for the loss of any single hand, foot, or eye. The maximum payable amount is $5,000.
