Employee Assistance Program (EAP)

The Employee Assistance Program (EAP)1 is self-funded by the Plan and is available for all participants. This benefit offers free, confidential help to eligible employees and their eligible family members with alcohol or drug problems (illegal or prescription).

Note that EAP benefits are not applied to deductibles or to annual or lifetime maximums for the Comprehensive Medical Benefit.

 

Deductible

EAP Referral: No Deductible
In-Network: $100 Deductible
Out-Of-Network: $200 Deductible

 

Lifetime Maximum

Dollar Maximum - $10,000
Treatment Maximum - 2 Treatment Courses (not to exceed 6 months each)

 

Course of treatment that may include Screening, Referral, Detoxification, Residential recovery facility, and follow-up visits (maximum 5 per lifetime)

EAP Referral: $100 of contract rate, no deductible
In-Network: 90% of contract rate, after deductible
Out-Of-Network: 70% of usual, customary & reasonable, after Out-of-Network deductible.

 

1. EAP benefits are not applied to deductible, or annual or lifetime maximums. All treatments must be authorized through the Plan.
