General
  • Designation of Beneficiary Form - This Form is designed for Participants, not receiving a pension, who want to designate their beneficiary. Please complete this Form and return it to the Administrative Office if the Participant wants to update or designate his/her beneficiary.
  • Defined Benefit SPD - This is the Defined Benefit Summary Plan Description.
Pre-Retirement
Retirement
  • Request For Return To Retirement Status - If your pension benefits have been suspended due to your work in covered employment, use this form to request reinstatement of your monthly retirement benefit.
  • Retirement Application - This is the retirement application. It contains instructions about additional information, and such as proof of age, that you need to provide.
  • Direct Deposit Enrollment Form - This form allows you to have your monthly pension check directly deposited into your checking account.
  • Form W-4P - This form is used to determine your Federal tax withholding.
  • Forms of Payment - This describes the various forms which are available for you to receive your pension payments. You will select one of the available forms at retirement, and future payments to you and/or your beneficiary are then governed by the selection you have made.
  • Required Documents for Retirement Application - Information and/or documents required for processing your application for retirement
